Program Exclusions & Limitations
This program is a savings plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan
- For services for injuries covered under worker's compensation
- For treatment, which, in sole opinion of treating dentist or doctor, lies outside the realm of their capability
- For referrals to specialists
- For hospitalization or hospital charges of any kind
- For costs of dental care which is covered under automobile medical or medical insurance
- The 12 month membership fee is due in full upon joining
- Membership fees cannot be financed through Care Credit
- Payments are due when services are rendered
- Renewal payment is due at the beginning of the same month of each year
- Accepted payment forms are cash, check, or credit card
This plan is only honored at Childress Dental. This plan is not an insurance plan and cannot be used with any other dental office.
